Label Form Instructions

Print Certified Mail and Return Receipt information, addresses, AND Internet Postage in a single step. Just print, apply, and you're done!

This Certified Mail form can be used to send ONLY ONE mail piece. You also have the option of adding Return Receipt Service to confirm delivery of your Certified Mail

How to Use:

To send Certified Mail WITHOUT Return Receipt Service (No Return Receipt Card)

Complete mailing information found within the MAILING tab of the Stamps.com software. In the "Printing On" section, select Certified Mail Form – Stamps.com SDC-3900 . Print on the Certified Mail Envelope. Click here to verify the correct way to feed the form into your printer.




Step 1

Peel Certified Mail label with Postage Attached (1) and Sender's Address label (2) from section A and apply to the TOP EDGE of mail piece as shown.

Note: If the sender address is already printed on your mail piece you may tear off and discard Sender's Address (2).

Step 2

Peel Delivery Address label (3) and apply to mail piece.

Step 3

Label (4) is your receipt for this mail piece. If you need a record of this mailing, present this receipt with the mail piece at your local post office to be postmarked when you send your certified mail (optional).

The stamped receipt is your proof of mailing for all tracking purposes. If you don't need a receipt, simply discard this label.

To send Certified Mail WITH Return Receipt Service (Uses Return Receipt Card)

Complete mailing information found within the MAILING tab of the Stamps.com software. In the "Printing On" section, select Certified Mail Form – Stamps.com SDC-3900. Print on the Certified Mail Envelope. Select the Return Receipt check box under "Services". Click here to verify the correct way to feed the form into your printer.





Step 1

Peel Return Address label (3) and carefully affix to the back of Return Receipt Card in the white "RETURN TO" area.

Step 2

Fold and tear horizontal perforation (1).

Step 3

Fold and tear vertical perforation 2. DO NOT TEAR OFF LABELS (5) and (6) FROM RETURN RECEIPT CARD.

Step 4

Peel indicated adhesive areas at each end of Return Receipt Card and apply it to the TOP EDGE of the mail piece as shown.

Step 5

Label (4) is your receipt for this mail piece. If you need a record of this mailing, present this receipt with the mail piece at your local post office to be postmarked when you send your certified mail (optional).

The stamped receipt is your proof of mailing for all tracking purposes. If you don't need a receipt, simply discard this label.
